    Housing Rehabilitation or Replacement
    Community Development Block Grant Program
    Taylor County will receive sealed bids from pre-qualified contractors interested in providing construction services for the Countyís CDBG Housing Rehabilitation Program. This project will include the rehabilitation of one (1) single family structure and the demolition/new construction of a minimum of eight (8) single family structures in the unincorporated area of Taylor County. All work is being funded in whole or in part through CDBG contract number 15DB-OJ-03-72-01-H 18.
    Mandatory pre-bid conferences will be held onsite for each of the proposed projects starting October 5 through October 7, 2015. The pre-bid conferences will begin at 8:00 am, outside of the Perry Foley Airport Terminal Conference Room, located at 401 Industrial Park Drive, Perry FL 32348.
    Bid packages will be provided to contractors at the conference.

    A Public Opening of the Bids is scheduled for November 2, 2015 at 6:00 PM at 201 East Green Street, Perry, Florida, 32347. Bids will be opened during a regularly scheduled Board of County Commissioners meeting.

    NOTICE OF INTENT TO ISSUE PERMIT
    The Department of Environmental Protection (Department), gives notice of its intent to issue a Mitigation Bank/ Environmental Resource Permit (Permit No. 331638-001) to Florida Gulf Coastal, LLC, 2835 NW 41st Street, Suite 220, Gainesville, Florida 32606, to establish the Florida Gulf Coast Mitigation Bank (“FGCMB”) on a ~ 1,588 acre coastal site.
    The Florida Gulf Coast mitigation bank site is located east of Cedar Key, between SR 24 and the Gulf of Mexico, more specifically in Sections 2, 3, 9, 10, 11, 13, 14, 15, 17, 20, and 21, Township 15 South, Range 13 East, Levy County, Latitude (DMS) 29o 11’ 07” Longitude (DMS) 83o 00’ 00”, Class III waters, adjacent to the Big Bend Seagrass Aquatic Preserve.  The mitigation service areas include portions of Wakulla, Jefferson, Taylor, Dixie, Levy, Citrus, Hernando, Pasco, Lafayette, Gilchrist, Suwannee, Alachua, Marion and Sumter Counties.
    The majority of the FGCMB site is saltwater marsh complex proposed for preservation only.  The site also contains a mosaic of freshwater marsh, forested wetlands (hydric hammock), maritime hammock, and sand-pine scrub forests proposed for preservation.  Four unpaved fill road crossings will be removed, two of which are to be converted to low water crossings, to restore historic salt marsh tidal regime and vegetation.  Freshwater depression marshes with tree and woody shrub encroachment will be enhanced through mechanical removal and prescribed fire.  Upland scrub and mesic flatwoods with inappropriate community structure are also proposed for enhancement through mechanical treatment and prescribed fire, as appropriate for each site.  The former landfill site will be restored to scrub through planting and fire management.  Two mitigation service areas are proposed for the potential use of credits: one for freshwater credits and one for saltmarsh credits.
    The Department will issue the environmental resource permit, unless a timely petition for an administrative proceeding is filed pursuant to the provisions of Sections 120.569 and 120.57, F.S. On the filing of a timely and sufficient petition, this action will not be final and effective until further order of the Department. Because the administrative hearing process is designed to formulate final agency action, the hearing process may result in a modification of the agency action or even denial of the application.

    A person whose substantial interests are affected by the Department’s action may petition for an administrative proceeding (hearing) under Sections 120.569 and 120.57, F.S. Pursuant to Rule 28-106.201, F.A.C., a petition for an administrative hearing must contain the following information:
    (a) The name and address of each agency affected and each agency’s file or identification number, if known;
    (b) The name, address, any email address, any facsimile number, and telephone number of the petitioner; the name, address, and telephone number of the petitioner’s representative, if any, which shall be the address for service purposes during the course of the proceeding; and an explanation of how the petitioner’s substantial interests are or will be affected by the agency determination;
    (c) A statement of when and how the petitioner received notice of the agency decision;
    (d) A statement of all disputed issues of material fact. If there are none, the petition must so indicate;

    The petition must be filed (received by the Clerk) in the Office of General Counsel of the Department at 3900 Commonwealth Boulevard, Mail Station 35, Tallahassee, Florida 32399-3000.
    Also, a copy of the petition shall be mailed to the applicant at the address indicated above at the time of filing.
    In accordance with Rule 62-110.106(3), F.A.C., petitions for an administrative hearing by the applicant must be filed within 21days of receipt of this written notice. Petitions filed by any persons other than the applicant, and other than those entitled to written notice under Section 120.60(3), F.S., must be filed within 21 days of publication of the notice or within 21 days of receipt of the written notice, whichever occurs first. Under Section 120.60(3), F.S., however, any person who has asked the Department for notice of agency action may file a petition within 21 days of receipt of such notice, regardless of the date of publication.     Under Rule 62-110.106(4), F.A.C., a person whose substantial interests are affected by the Department’s action may also request an extension of time to file a petition for an administrative hearing. The Department may, for good cause shown, grant the request for an extension of time. Requests for extension of time must be filed with the Office of General Counsel of the Department at 3900 Commonwealth Boulevard, Mail Station 35, Tallahassee, Florida 32399-3000, before the applicable deadline for filing a petition for an administrative hearing. A timely request for extension of time shall toll the running of the time period for filing a petition until the request is acted upon.

    package marked “Sealed Qualifications for MISDEMEANANT OFFENDER SUPERVISION
    SERVICES” to the Clerk of Court, 1st Floor Courthouse, 108 North Jefferson
    Street, Suite 102, or P.O. Box 620, Perry, Florida 32348, to arrive no later
    than 4:00 P.M., local time, on Friday, October 16, 2015.  All qualifications
    MUST have the respondent’s name and mailing address clearly shown on the
    outside of the envelope or package when submitted.  Qualifications will be
    opened and respondents announced at 6:00 P.M. local time, or as soon
    thereafter as practical, on Tuesday, October 20, 2015, in the Taylor County
    Administrative Complex, Old Post Office, 201 East Green Street, Perry,
    Florida 32347.
    Qualification information MUST be obtained from the Office of the County
    Administrator located at the Administrative Complex, 201 East Green Street,
    Perry, Florida 32347.

    Qualifications must include the following tabbed sections:

    3.    Staff (Personnel to be responsible for performing services and
    accompanying resumes).
    4.    References for similar projects in past 5 years
    5.    Project Approach

    7.    Required Forms (see pg. #5)
    The County reserves the right, in its sole and absolute discretion, to
    reject any or all qualifications, to cancel or withdraw this request for
    qualifications at any time and waive any irregularities in the qualification
    process.  The County reserves the right to award any contract to the
    respondent which it deems to offer the best overall service; therefore, the
    County is not bound to award any contract based on the lowest quoted price.
    The County, in its sole and absolute discretion, also reserves the right to
    waive any minor defects in the process and to accept the qualification
    deemed to be in the County’s best interest. The County, in its sole and
    absolute discretion, also reserves the right to assign a local business
    preference in a maximum amount of five (5) percent of the total score, under
    the conditions set forth in Ordinance 2003-12.  No faxed qualifications will
    be accepted.
