Shirley Ann Allen, 67, of Ashland, Ky., formerly of Perry, died April 29, 2013, at King’s Daughters Medical Center in Ashland.

The memorial service will be May 1 at 7 p.m. at Faith Christian Assembly in Flatwoods, Ky. Burial will follow at White City Cemetery in Ft. Pierce.
