    NOTICE OF PUBLIC HEARING
    CONCERNING AN AMENDMENT TO THE CITY OF PERRY LAND DEVELOPMENT REGULATIONS
    BY THE PLANNING AND ZONING BOARD OF THE CITY OF PERRY, FLORIDA, SERVING ALSO AS THE LOCAL PLANNING AGENCY OF THE CITY OF PERRY, FLORIDA, NOTICE IS HEREBY GIVEN that, pursuant to Sections 163.3161 through 163.3248, Florida Statutes, as amended, and the City of Perry Land Development Regulations, as amended, hereinafter referred to as the Land Development Regulations, objections, recommendations and comments concerning the amendment, as described below, will be heard by the Planning and Zoning Board of the City of Perry, Florida, serving also as the Local Planning Agency of the City of Perry, Florida, at a public hearing on June 4, 2018 at 5:30 p.m., or as soon thereafter as the matter can be heard, in the City Council Meeting Room, City Hall located at 224 South Jefferson Street, Perry, Florida.
    LDR 18-01, an application by the City Council to amend the text of the Land Development Regulations by amending Section 4.19.2 entitled “PRD” Planned Residential Development Permitted Principal Uses and Structures to add assisted living facilities, memory car facilities, nursing homes and similar uses.
    The public hearing may be continued to one or more future dates.  Any interested party shall be advised that the date, time and place of any continuation of the public hearing shall be announced during the public hearing and that no further notice concerning the matter will be published, unless said continuation exceeds six calendar weeks from the date of the above referenced public hearing.
    At the aforementioned public hearing, all interested parties may appear to be heard with respect to the amendment.
    Copies of the amendments are available for public inspection at the Office of the City Manager, City Hall located at 224 South Jefferson Avenue, Perry, Florida, during regular business hours.
    All persons are advised that if they decide to appeal any decision made at the above referenced public hearings, they will need a record of the proceedings, and that for such purpose they may need to ensure that a verbatim record of the proceedings is made, which record includes the testimony and evidence upon which the appeal is to be based.

    Public Notice of Intent to Issue Air Permit

    Applicant: The applicant for this project is Foley Cellulose, LLC. The applicant’s authorized representative and mailing address is: Lee Davis, Vice President-General Manager, Foley Cellulose, LLC, Foley Mill, One Buckeye Drive, Perry, Florida 32348.
    Facility Location: Foley Cellulose, LLC operates the existing Foley Mill, which is in Taylor County at One Buckeye Drive in Perry, Florida.
    Project: The Foley Mill is an existing softwood Kraft Process Pulp Mill that manufactures bleached market pulps and dissolving cellulose pulps consisting of the following major activities: two Kraft pulp mills, chemical recovery, causticizing, purification (i.e. bleaching), papermaking, woodyard, and utility operations.
    The purpose of this project is to produce a new pulp product on the No. 2 Pulping line. The project will consist of decoupling the Line 3 process from the No. 2 Pulping Line, and modifying and installing new equipment. Emission sources from the project consist of the following: No. 2 Batch Digester System; Conveyor C-11 extension; new Repulper; increase in noncondensable gases to the No. 1 Bark Boiler (primary control) and No. 1 Power Boiler (backup control); and increase in black liquor solids (recovery furnaces, smelt dissolving tanks and lime kiln). Emissions increases from the project consist of: 24.95 tons per year (TPY) for carbon monoxide; 26.02 TPY for nitrogen oxides; 18.11 TPY for particulate matter (PM); 11.64 TPY for PM with a mean diameter of 10 microns or less (PM10); 5.76 TPY of PM2.5; 36.53 TPY of sulfur dioxide (SO2); 1.42 TPY of sulfuric acid mist; 12.05 TPY of volatile organic compounds; 0.002 TPY of lead; 5.92 TPY of total reduced sulfur (TRS); 2.09 TPY of hydrogen sulfide; and 2,133.97 TPY of greenhouse gases. To provide reasonable assurance that the project does not trigger Prevention of Significant Deterioration (PSD) review pursuant to Rule 62-212.400, Florida Administrative Code, the applicant is required to monitor and report for a period of 5-years the actual pulp production, steam demand, and actual emissions reporting for SO2 and TRS.
    Permitting Authority: Applications for air construction permits are subject to review in accordance with the provisions of Chapter 403, Florida Statutes (F.S.) and Chapters 62-4, 62-210 and 62-212 of the Florida Administrative Code (F.A.C.). The proposed project is not exempt from air permitting requirements and an air permit is required to perform the proposed work. The Permitting Authority responsible for making a permit determination for this project is the Office of Permitting and Compliance in the Department of Environmental Protection’s Division of Air Resource Management. The Permitting Authority’s physical address is: 2600 Blair Stone Road, Tallahassee, Florida.     Notice of Intent to Issue Air Permit: The Permitting Authority gives notice of its intent to issue an air construction permit to the applicant for the project described above. The applicant has provided reasonable assurance that operation of proposed equipment will not adversely impact air quality and that the project will comply with all applicable provisions of Chapters 62-4, 62-204, 62-210, 62-212, 62-296 and 62-297, F.A.C. The Permitting Authority will issue a Final Permit in accordance with the conditions of the proposed Draft Permit unless a timely petition for an administrative hearing is filed under Sections 120.569 and 120.57, F.S. or unless public comment received in accordance with this notice results in a different decision or a significant change of terms or conditions.
    Comments: The Permitting Authority will accept written comments concerning the proposed Draft Permit for a period of 14 days from the date of publication of the Public Notice. Written comments must be received by the Permitting Authority by close of business (5:00 p.m.) on or before the end of this 14-day period. If written comments received result in a significant change to the Draft Permit, the Permitting Authority shall revise the Draft Permit and require, if applicable, another Public Notice. All comments filed will be made available for public inspection.
    Petitions: A person whose substantial interests are affected by the proposed permitting decision may petition for an administrative hearing in accordance with Sections 120.569 and 120.57, F.S. Petitions filed by any persons other than those entitled to written notice under Section 120.60(3), F.S., must be filed within 14 days of publication of the Public Notice or receipt of a written notice, whichever occurs first. Under Section 120.60(3), F.S., however, any person who asked the Permitting Authority for notice of agency action may file a petition within 14 days of receipt of that notice, regardless of the date of publication. A petitioner shall mail a copy of the petition to the applicant at the address indicated above, at the time of filing. A petition for administrative hearing must contain the information set forth below and must be filed (received) with the Agency Clerk in the Office of General Counsel, 3900 Commonwealth Boulevard, MS 35, Tallahassee, Florida 32399-3000, Agency_Clerk@dep.state.fl.us, before the deadline. The failure of any person to file a petition within the appropriate time period shall constitute a waiver of that person’s right to request an administrative determination (hearing) under Sections 120.569 and 120.57, F.S., or to intervene in this proceeding and participate as a party to it. Any subsequent intervention (in a proceeding initiated by another party) will be only at the approval of the presiding officer upon the filing of a motion in compliance with Rule 28-106.205, F.A.C.
    A petition that disputes the material facts on which the Permitting Authority’s action is based must contain the following information: (a) The name and address of each agency affected and each agency’s file or identification number, if known; (b) The name, address, any email address, telephone number and any facsimile number of the petitioner; the name, address any email address, telephone number, and any facsimile number of the petitioner’s representative, if any, which shall be the address for service purposes during the course of the proceeding; and an explanation of how the petitioner’s substantial interests will be affected by the agency determination; (c) A statement of when and how each petitioner received notice of the agency action or proposed decision; (d) A statement of all disputed issues of material fact. If there are none, the petition must so state; (e) A concise statement of the ultimate facts alleged, including the specific facts the petitioner contends warrant reversal or modification of the agency’s proposed action; (f) A statement of the specific rules or statutes the petitioner contends require reversal or modification of the agency’s proposed action including an explanation of how the alleged facts relate to the specific rules or statutes; and, (g) A statement of the relief sought by the petitioner, stating precisely the action the petitioner wishes the agency to take with respect to the agency’s proposed action. A petition that does not dispute the material facts upon which the Permitting Authority’s action is based shall state that no such facts are in dispute and otherwise shall contain the same information as set forth above, as required by Rule 28-106.301, F.A.C.
    Because the administrative hearing process is designed to formulate final agency action, the filing of a petition means that the Permitting Authority’s final action may be different from the position taken by it in this Public Notice of Intent to Issue Air Permit. Persons whose substantial interests will be affected by any such final decision of the Permitting Authority on the application have the right to petition to become a party to the proceeding, in accordance with the requirements set forth above.
    Mediation: Mediation is not available in this proceeding.

    Taylor County Schools will be participating in the Summer Food Service Program during the months of June and July.
    Nutritionally balanced meals will be provided to all children regardless of race, color, sex, disability, age or national origin during summer vacation when school breakfasts and lunches are not available. All children 18 years old and younger, if open site, are eligible for meals at no charge, and there will be no discrimination in the course of the meal service. The programs are only approved for geographical areas of need where 50 percent or more of the children qualify for free and reduced-price meals during the school year.

    NOTICE OF REQUEST FOR PROPOSALS FOR OLD HOSPITAL ASBESTOS ABATEMENT
    The Taylor County Board of County Commissioners is soliciting sealed proposals for asbestos abatement/removal on the 2-story medical building and 1-story support services building at the Taylor County Old Hospital site located at 407 E Ash Street, Perry, Florida 32347.
    Qualified firms or individuals desiring to provide the required services must submit the proposal packages in a sealed envelope or similar package marked “Sealed Proposal for Old Hospital Asbestos Abatement” to the Clerk of Court, 1st Floor Courthouse, 108 North Jefferson Street, Suite 102, Perry, Florida 32347, to arrive no later than 4:00 P.M., local time, on Friday, July 13, 2018. All proposals MUST have the respondent’s name and mailing address clearly shown on the outside of the envelope or package when submitted. Proposals will be opened and respondents announced at 9:00 A.M. local time, or as soon thereafter as practical, on Tuesday, July 17, 2018, in the Taylor County Administrative Complex, Old Post Office, 201 East Green Street, Perry, Florida 32347.

    3. Scope of Work – Proposal detail. Refer to Asbestos Renovation Survey available from the Taylor County Administrative Complex for suggested scope of work.
    4. Fee – Proposed pricing for asbestos abatement and any required demolition.
    5. Project Timeline – Provide timeline outlining proposed starting date, estimated length of project, and date of completion.
    The County reserves the right, in its sole and absolute discretion, to reject any or all proposals, to cancel or withdraw this request for proposals at any time and waive any irregularities in the proposal process. The County reserves the right to award any contract to the respondent which it deems to offer the best overall service; therefore, the County is not bound to award any contract based on the lowest quoted price. The County, in its sole and absolute discretion, also reserves the right to waive any minor defects in the process and to accept the proposal deemed to be in the County’s best interest. The County, in its sole and absolute discretion, also reserves the right to assign a local business preference in a maximum amount of five (5) percent of the proposed price(s), under the conditions set forth in Ordinance 2003-12. No faxed proposals will be accepted.
