What are the health care needs of our community?

The Florida Department of Health of Taylor County (DOH-Taylor) and Doctors’ Memorial Hospital (DMH) want to know how they can better serve their patients and the community.

Two of Taylor County’s main health care providers are partnering together to update their “Community Health Needs Assessment,” are asking residents to participate in a survey designed to improve the health of the community.

This assessment is completed every three years to better understand what issues local residents face and what the health department, hospital and local community organizations should be working on to better address the region’s healthcare needs.

To provide residents the opportunity to give their feedback and thoughts on area healthcare needs, a short survey has been developed and is available online at https://forms.office.com/r/Efi7qU9ir0.

The online survey takes less than 10 minutes to complete. Responses are kept confidential, and the deadline to participate is Sunday, May 1, 2022.

Feedback from the survey and other informational gathering processes will be used to establish a Community Health Needs Assessment Implementation Plan.
